Application Form

To apply for participating in JVL Summer School for Performing Arts, please do the following:

1. Download and print the forms using one of the links below;
2. Fill out and sign the Application Form and the Terms and Conditions / Waiver / Medical Form;
3. Mail the forms together with the deposit cheque(s) to the address indicated in the Application Form.

Alternatively, you can submit your Application Form on-line. To apply on-line, please do the following:

1. Open On-Line Application page;
2. Fill out and submit the Application Form
3. Pay fees online with Credit Card / PayPal
